Can an implant be placed immediately after extraction?

Sometimes. “Immediate implant” means the implant is placed in the extraction socket at the same appointment. It does not automatically mean a final tooth is attached immediately or that the implant is immediately put into full chewing function.

SHORT ANSWER

Sometimes. “Immediate implant” means the implant is placed in the extraction socket at the same appointment. It does not automatically mean a final tooth is attached immediately or that the implant is immediately put into full chewing function.

What makes immediate placement possible?: The socket anatomy, remaining bone walls, soft tissue, infection control, ability to place the implant in the correct three-dimensional position and the ability to achieve adequate primary stability all matter.

What makes immediate placement possible?

The socket anatomy, remaining bone walls, soft tissue, infection control, ability to place the implant in the correct three-dimensional position and the ability to achieve adequate primary stability all matter.

Infection is not a simple yes/no rule

Selected sites with chronic periapical pathology may still be considered for immediate placement when the area can be thoroughly debrided and stability can be achieved. Acute or uncontrolled infection requires different judgement.

The plan can change during extraction

A case may be planned for immediate implant placement but converted to delayed placement if the socket is more damaged than expected or adequate stability cannot be achieved. That is a normal contingency, not a failed plan.

International timing

Do not book a short trip on the assumption that same-day implant placement will definitely be possible. The extraction itself can provide new information that changes the sequence.

Schematic illustration

Immediate placement does not stop socket remodelling

Placing an implant immediately after extraction does not preserve the entire socket contour by itself. Bone and soft tissue still remodel, and grafting around the implant may be considered depending on the gap and tissue phenotype.

Aesthetic-zone cases require particular care

In visible anterior areas, facial bone thickness, gum phenotype, implant position and provisional restoration design influence soft-tissue stability. A technically successful implant can still produce an aesthetic problem if placed too far facially or in the wrong vertical position.

Frequently asked questions

Is immediate implant placement more successful than delayed placement?

In well-selected cases both can perform well. The advantage of immediate placement is usually treatment sequencing, not proven universal superiority.

Can I have an implant if there was an infection?

Selected chronic infection does not automatically exclude immediate placement when the site can be debrided and stability achieved. Acute uncontrolled infection is a different situation.

Primary stability and socket integrity

Immediate placement often relies on engaging bone beyond or around the extraction socket to achieve stability. If that bone is absent or the socket is damaged, placing an implant simply to preserve the original schedule may create a poorer position or higher risk.

Why a delayed implant is not second-class treatment

Allowing soft tissue or bone to heal first can simplify some cases and provide clearer tissue conditions. “Immediate” is a timing protocol, not a quality grade.

A same-day plan needs a backup plan

Before extraction, agree how the gap and extraction site would be managed if immediate placement proved unsuitable. The fallback may change the temporary tooth, grafting discussion or timing of the next visit. Those possibilities should be part of the discussion before an irreversible procedure starts.

If you are travelling from abroad

Ask what happens if immediate placement is not possible. Will the site be grafted? Will a temporary tooth be provided? When would the next visit be expected? Clarifying the alternative pathway before travel prevents a same-day marketing phrase from becoming an unrealistic promise.
